Q-Switched vs Pico Laser: Which One Does Your Pigmentation Need?

Pico lasers arrived in Indian clinics with a great deal of marketing, and patients now ask for one by name. It is a genuinely good technology. It is also not automatically the right answer, and a clinic that recommends it for everything is doing marketing rather than dermatology. Here is what actually separates the two.

The Real Difference: Time, Not Power

Both lasers target melanin. What differs is how long each pulse lasts.

A Q-switched laser fires in nanoseconds — billionths of a second. That is fast enough to shatter pigment while limiting heat spread into surrounding tissue, which is why it became the standard for pigmentation on darker skin in the first place.

A picosecond laser fires in picoseconds — trillionths of a second, roughly a thousand times shorter again. At that speed the dominant mechanism changes. Instead of primarily heating pigment until it fragments, the pulse creates a rapid pressure wave that shatters it mechanically. This is called the photomechanical effect, and it has two practical consequences: pigment breaks into smaller particles that the immune system clears more readily, and less heat is deposited in the surrounding skin.

That second point is the one that matters most on Fitzpatrick IV–VI skin. Heat is what drives post-inflammatory hyperpigmentation. Less thermal load means less of the thing that causes the complication patients most want to avoid.

Tattoo Removal Is Where the Gap Is Widest

If there is one application where the picosecond advantage is not marketing, it is tattoo removal. Ink particles are large and stubborn, and mechanical shattering outperforms thermal fragmentation clearly. Patients typically need fewer sessions, and the colours that historically frustrated everyone — green, sky blue, turquoise — respond considerably better.

For pigmentation the gap is narrower and more situational, which is the part the advertising tends to skip.

The Melasma Caveat Worth Knowing

Patients often arrive asking for Pico specifically for melasma, having read that it is the newer and therefore better option. The honest position is more restrained. Melasma is not simply deposited pigment — it is an ongoing overproduction driven by hormones, heat, UV, and visible light. No laser switches that off.

Both platforms can clear existing pigment. Neither prevents recurrence. Melasma treated aggressively with any device — picosecond included — can rebound worse than the starting point. If a clinic tells you Pico cures melasma, that is a reason for caution, not confidence.

What Does Not Change Between Them

Both lasers are only as safe as the settings and the hands. On darker skin the failure mode is identical — too much energy, too short an interval, no test patch, no sun protection afterwards. Both require a course rather than a single session. Both need the pigment correctly diagnosed first, because treating melasma as though it were sun spots produces a poor outcome on either device.

Device choice is genuinely the smaller half of the decision. Diagnosis and calibration are the larger half.

How We Choose

We run both platforms and select per patient rather than per package. Q-switched Nd:YAG remains the mainstay for routine toning and melasma maintenance, where its low-fluence protocols are well proven on Indian skin. We reach for the picosecond laser when pigment has resisted nanosecond toning, for tattoo work, and where reducing thermal load is the priority on more reactive skin.

Conservative fluence, adequate intervals, and sun protection matter on both lasers.

How many sessions will I need?

For pigmentation, typically 6–10 toning sessions spaced 2–3 weeks apart on either platform, supported by topicals and daily SPF.
